Getting Ready for Your Visit
In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- Drivers License or Photo ID Card
- Insurance Card
- List of all prescription medicines you take
- List of over-the-counter medications
- List of Allergies
- Completed Patient Registration Forms (also available at the front desk)
Billing & Insurance
We accept most insurance plans. If you have questions, please contact your insurance company to see if we are in network. All insurance co-payments and deductibles are due at the time of service and will be collected at check-in. We accept cash, Visa, and MasterCard as payment options. We are sorry, but we do not accept checks. Please remember to bring your insurance card with you every time you visit us for an appointment.
