Our weight loss program offers physician supervised medically assisted treatment and an individualized approach to weight management. We help our patients make sustainable lifestyle changes through counseling and education.

What to expect during the first visit
Arrive 15-20 minutes before the scheduled appointment to fill out our weight loss intake form and consent forms. (These forms can also be found at the bottom of this page).
A medical assistant will take your weight and vital signs. You will spend at least 30 minutes with one of our providers (our team consists of nurse practitioners and physician’s assistants under the supervision of Dr. Daniel Seward). During that time, we will discuss your past medical history, your goals, cravings and eating patterns, lifestyle habits, and medically assisted treatment options. The provider will complete a physical exam, and then our medical assistants will take a blood sample for laboratory work.
Patients will be seen by appointment only.
After the first visit, you will be seen by a provider monthly for weight loss follow up. Some of the weight loss medications we use are controlled medicines and require monthly appointments in order to receive the script.
Payment options:
Insurance will be accepted for primary care patients. Those patients with closed-network insurance plans will have to pay the out of pocket fees.

Includes laboratory work, education about approved weight loss medications, and provider assisted selection and prescription of medicine*
*Based on patient qualification, the patient is responsible for the cost of the medication
